Purpose of the Program & Test

The Wild Roots Gut Reset Program includes the GI-MAP® Stool Test, a functional laboratory test designed to provide insight into digestive and microbiome health for wellness and educational purposes.

This test and all associated services are not intended to diagnose, treat, cure, or prevent any disease.

All information provided by Wild Roots Nutrition — including lab interpretation, protocols, and recommendations — is educational in nature and does not replace care from a licensed medical provider.

Scope of Practice

Wild Roots Nutrition operates under a functional nutrition model. Services are designed to support overall health through nutrition, lifestyle, and supplement guidance.

Jamie Pantano is a Certified Nutritional Therapy Practitioner and is not a licensed medical provider.

You agree to consult your physician or qualified healthcare provider regarding any medical condition, medication adjustments, or health concerns.

Program Overview

The Wild Roots Gut Reset Program includes:

• One (1) GI-MAP® Stool Test kit (shipped directly to client; not available to residents of NY, NJ, or RI)

• Lab processing through Diagnostic Solutions Laboratory

• Intake forms + 3-day food & mood journal

• One (1) 90-minute Results & Strategy Session

• One (1) 30-minute Follow-Up Session

• One (1) 30-minute Wrap Up Session

• Eight (8) weeks of personalized support

• Ongoing email/text support

• Customized nutrition, lifestyle, and supplement protocol based on lab findings

All live sessions must be completed within four (4) months of purchase.

Client Responsibilities

By enrolling, you agree to:

• Confirm you do not reside in New York, New Jersey, or Rhode Island

• Follow all test kit instructions carefully

• Submit your stool sample within 30 days of receiving the kit in the mail

• Complete all intake forms before results review

• Provide accurate and complete health information

• Consult with your physician regarding medical concerns

• Communicate openly regarding symptoms and progress

Failure to complete required paperwork may delay your results session.

Lab Timing & Protocol Preparation

After your sample is received by the lab, results typically take 2–3 weeks.

Once results are received:

• All intake paperwork must be complete before review

• It may take 5–7 business days to prepare your personalized protocol

• Your 90-minute session will be scheduled once preparation is complete

Investment & Payment

Program Investment: $895
(Initial promotional pricing may apply for limited enrollment periods.)

Payment is due in full before the test kit is ordered.

Charges will appear as Wild Roots Nutrition, LLC.

Refund Policy

Due to the cost of laboratory testing:

• Once the GI-MAP® kit has been ordered, the program is non-refundable.

• If a test kit is shipped but not completed within 30 days, it may expire and require repurchase.

• No refunds will be issued after the Results & Strategy Session has occurred.

• Missed sessions without 48 hours’ notice may be forfeited.

Wild Roots Nutrition reserves the right to discontinue services at its discretion for client safety or scope-of-practice concerns.

Privacy & Data Handling

Wild Roots Nutrition collects personal and health information solely for the purpose of providing services.

While Wild Roots Nutrition is not a HIPAA-covered entity, reasonable administrative and technical safeguards are used to protect client data stored electronically.

By enrolling, you acknowledge and accept the inherent risks of electronic communication and consent to receive materials and correspondence digitally.

Your information will not be shared without your consent except as required by law.

Limitation of Liability

By participating in this program, you assume full responsibility for your health decisions and actions.

Wild Roots Nutrition, LLC and Jamie Pantano are not liable for any direct or indirect damages resulting from the use or misuse of information provided.

Governing Law

This agreement shall be governed by and construed in accordance with the laws of the State of North Carolina.
