The GI-MAP® Stool Test is a functional lab test designed to provide insight into gut health for wellness and educational purposes. It is not intended to diagnose, treat, or cure any medical condition.

All information shared through Wild Roots Nutrition, including test interpretation and recommendations, is not medical advice and should not replace care from your primary healthcare provider.

Scope of Practice

Wild Roots Nutrition operates under a functional nutrition model. Services provided are intended to support overall health through nutrition and lifestyle guidance, not to diagnose or treat disease. If you have a medical concern, you agree to consult with your licensed physician or healthcare provider.

What’s Included

Your purchase includes:

• One (1) GI-MAP® Stool Test kit shipped to your preferred address

• Lab processing and results through Diagnostic Solutions Laboratory

• A 60-90 minute results review session with a Certified Nutritional Therapy Practitioner

• A personalized protocol and written recommendations (PDF)

Note: Follow-up sessions, additional testing, and supplements are not included unless otherwise stated.

Client Responsibilities

You are responsible for:

• Carefully following all test kit instructions

• Shipping your sample to the lab in a timely manner

• Providing accurate and complete intake information

• Consulting with your doctor about any findings or health concerns

Refunds & Cancellations

Once your test kit has been ordered or shipped, no refunds will be issued. If your test is not completed or submitted within 6 months, it may be voided and a new kit will need to be purchased at full price

Privacy & Confidentiality

Your health data and test results will be handled with care and kept confidential in accordance with HIPAA-compliant practices. Results will only be shared with Wild Roots Nutrition for the purpose of providing services.

Consent to Services

By completing your purchase, you consent to receive functional nutrition services from Wild Roots Nutrition, including the review of your GI-MAP® test results and the creation of a wellness plan. You understand this service is for educational purposes and not medical advice.
